On this date, , Lisa Glasco(seller) agrees to sell a male/female German Shepherd puppy from litter# with microchip# , sired by: (Sire) and out of (Dam) whelped on , for the sum of $ .
Health Guarantee:
Carisa Shepherds (Jim and Lisa Glasco), hereinafter referred to as sellers, guarantees this puppy to be a purebred German Shepherd Dog eligible for registration with the AKC. Puppy is guaranteed to be healthy and free of communicable diseases at time of transfer.
Buyer is encouraged to have the puppy examined by a licensed veterinarian at buyer's expense within 3 business days of taking possession of puppy. Should the puppy be found to be unhealthy within the first 3 business days, buyer can return the puppy to sellers for a full refund. Also, if the puppy is found unsatisfactory for any reason during this period, buyer can return the puppy for a full refund or replacement puppy if one is available.
Should puppy be found unhealthy, buyer must provide a veterinary statement as to the nature of the problem. Buyer is responsible for all return transportation expenses. All paperwork must be returned to sellers and signed to transfer ownership back to sellers. Sellers are in no way responsible for any veterinarian expenses incurred by buyer.

Hereditary Defects Guarantee:
Seller guarantees puppy to the age of 36 months to be free of any crippling genetic defects that would prevent puppy from living a normal, healthy life. Should any defect be found during this time, seller will replace puppy with another puppy of equal value when one becomes available. Buyer must provide a signed statement from a licensed veterinarian detailing the nature of the defect and, in the case of hip dysplasia, provide x-rays to the seller to qualify for replacement. Seller reserves the right to have the dog examined by veterinarian of seller's choice at seller's expense.
Puppy is also guaranteed to be able to have hips and elbows certified by OFA at 24 months with a fair, good or excellent rating. Should the puppy not obtain a fair, good or excellent rating, seller will replace puppy with a puppy of equal value when one becomes available. Buyer must provide proof of OFA rating and copies of the x-rays sent to OFA to qualify for replacement.
Should a replacement be required, buyer has two options as to the original puppy. Buyer may keep original puppy and provide proof of spay/neuter of said puppy before receiving a replacement. The buyer may also return the original puppy at buyer's expense to sellers. If buyer chooses to return original puppy, buyer must sign all paperwork needed to transfer ownership of said puppy to sellers.
All veterinary and transportation expenses are the responsibility of the buyer except in the event that the sellers choose to have the puppy examined by a veterinarian of seller's choice which will be at seller's expense.
This is the entire guarantee and no other guarantee is expressed or implied.
